Abstract
Un objetivo del médico es identificar a las personas que tienen un mayor riesgo de evolucionar negativamente, actuar precozmente y anticiparnos a ese empeoramiento o deterioro con el fin de estabilizar o revertir el proceso y mantener a nuestro paciente en un estado de “salud”. La medicina basada en la evidencia y los diferentes estudios científicos realizados permiten, en determinados procesos, conocer cuáles son los indicadores de gravedad, clasifican al paciente y dan margen para anticiparnos y actuar de una manera precoz planificando los cuidados necesarios. Nuestro objetivo con este trabajo es ampliar el campo, dentro de nuestras posibilidades, para conocer sobre “quienes” tenemos que actuar precozmente con el fin de conseguir aumentar su supervivencia. Las personas institucionalizadas presentan un perfil de fragilidad, con alta dependencia y necesidad de cuidados, tanto desde el punto de vista médico y de enfermería como del personal auxiliar. Durante su ingreso van a sufrir enfermedades crónicas, procesos morbosos y complicaciones que empeoran su situación y las hacen más vulnerables. La decisión del ingreso en una residencia la mayoría de las veces viene condicionada por problemas económicos y fundamentalmente apoyada por la incertidumbre del tiempo que tendrán que afrontarlo. Conocer el periodo, o una expectativa aproximada, podría ayudar, no sólo a que la decisión sea tomada con mayor seguridad en el aspecto económico, sino también permitiría desarrollar más ampliamente productos aseguradores o financieros que faciliten el alto coste de estos recursos. Por tanto, conocer las variables que pueden determinar el pronóstico de mortalidad, y a su vez una expectativa de vida, puede proporcionar un elemento clave para tomar una decisión en la institucionalización de la persona dependiente...
One of the goals of the physician is to identify people who have a higher risk of evolving negatively, to act preemptively, and to anticipate this deterioration in order to stabilize or reverse the process and to keep our patient in a state of “health”. Evidence-based medicine and the different scientific studies allow us in certain processes to recognize the indicators, classify the patient, and give us time to anticipate and act early, planning the care needed. Our objective with this essay is to increase the field of our possibilities in order to know when and where to act preemptively in order to raise the survival rate. The institutionalized people show a profile of fragility, high dependency and need of care, from a medical, nursing and auxiliary personnel perspective. During the stay they are going to suffer chronic diseases, morbid processes and complications that worsen their basal situation and make them more vulnerable. The decision of admission into a residential center in most cases is determined by economic issues and fundamentally on the uncertainty of how much time the situation will have to be dealt with. Knowing the period or the approximate expectation could help not only in order to make the decision with more certainty economically, but it would also allow for the development of more financial or insurance products that will ease the high cost of these resources. Therefore, knowing the variables that can determine the mortality prognosis, and subsequently a life expectancy, may provide a key element to the decision-making process about the institutionalization of a dependent person.
