Abstract
La Nefropatía IgA (NIgA) es la enfermedad glomerular primaria más prevalente a nivel mundial. Inicialmente era considerada como una enfermedad relativamente benigna, pero ahora se sabe que existe un riesgo sustancial de progresión a la enfermedad renal terminal (ERT). El índice de progresión a los diez años es del 5 - 25% y a los veinte años del 25 - 50%. Además la NIgA afecta a un gran porcentaje de gente joven en comparación con otras enfermedades que llevan a la enfermedad renal crónica y a la necesidad de tratamiento renal sustitutivo, con la consecuente implicación económica y social que esto conlleva. Por todos estos motivos es importante identificar aquellos factores demográficos, clínicos e histológicos que nos puedan ser útiles para predecir el pronóstico de los pacientes con NIgA, lo cual va a ser determinante a la hora de decidir el manejo clínico y terapéutico más adecuado de estos enfermos. De forma preferente, es preciso identificar los marcadores pronósticos en la subpoblación de pacientes con NIgA progresiva, por la morbimortalidad que la progresión de la enfermedad renal conlleva. Además, la evidencia científica existente acerca del tratamiento óptimo de los pacientes con NIgA, especialmente en este subgrupo de pacientes con NIgA y evidencia de progresión de la enfermedad, es muy escasa. Son necesarios más estudios que exploren nuevas estrategias terapéuticas de forma preferencial en estos pacientes con NIgA progresiva, por su comorbilidad asociada y porque es en este tipo de pacientes en los que se va a plantear el empleo de fármacos inmunosupresores agresivos y potencialmente tóxicos...
IgA nephropathy (IgAN) is the leading cause of glomerulonephritis worldwide. Although it has traditionally been considered a relatively benign disease, we nowadays know that there is a substantial risk of progression to end-stage renal disease (ESRD). This risk of progression is 5-25% at ten years, and 25-50% at twenty years. Moreover, IgAN affects young people predominately, with the socioeconomic impact that this entails, in contrast with other kidney diseases that lead to ESRD and renal replacement therapy. For all these reasons, we need studies that focus on the detection of demographic, clinical and histological factors which predict progression of this disease. Those predictors will be extremely useful in the daily clinical practice, in order to decide the most convenient therapeutic management. We must identify preferentially the prognostic factors in the subpopulation of patients with progressive IgAN, because of the morbidity and mortality that progressive renal disease entails. In addition, there is lack of evidence about the optimal treatment in IgAN patients, especially in those patients with progressive disease. We need more studies exploring new therapeutic strategies, mainly in those patients with progressive IgAN, because of their inherent morbid-mortality and the potential toxicity of the immunosuppressive therapies used in this group of patients...
