Efectos del tratamiento periodontal en los marcadores de inflamación sistémicos y locales de pacientes con síndrome metabólico: un ensayo clínico controlado

Abstract
Objetivos. El objetivo principal del estudio fue evaluar el efecto del tratamiento periodontal no quirúrgico sobre los marcadores inflamatorios sistémicos en pacientes con SMet, analizando los niveles de CRP, Alfa-1 antitripsina y fibrinógeno en plasma. Los objetivos secundarios fueron determinar el impacto del tratamiento periodontal sobre el metabolismo de los lípidos y carbohidratos, mediante el análisis de los cambios en la hemoglobina glicosilada (HbA1c) en plasma, los triglicéridos, el colesterol sérico total, lipoproteínas de alta densidad (HDL) y lipoproteínas de baja densidad (LDL). Material y Métodos. Se realizó un ensayo clínico aleatorizado, con doble ciego y una duración de 6 meses a pacientes con periodontitis y síndrome metabólico. Los participantes fueron seleccionados de manera aleatoria para su inclusión en el grupo tratamiento (n= 7), al que se le realizó un tratamiento periodontal intensiva (IPT) o en el grupo control (n= 9), donde se llevó a cabo un tratamiento periodontal mínimo (MPT). Resultados. Se realizó examen periodontal a 151 pacientes con diagnóstico de síndrome metabólico. De los cuales un total de 20 llegaron a ser objeto de aleatorización y 16 de ellos asistieron a todas las visitas del estudio. Se encontraron diferencias significativas entre los grupos a nivel basal en proteína C reactiva, alfa-1 antitripsina y fibrinógeno (p≤0.05). No se encontraron diferencias estadísticamente significativas entre ambos grupos en los valores de proteína C reactiva, alfa-1 antitripsina y fibrinógeno, no obstante, la diferencia en la disminución de la proteína C reactiva mostró un valor cercano a la significación estadística (p= 0.698) Conclusión La disminución de los valores de marcadores inflamatorios sistémicos (proteína C reactiva, alfa-1 antitripsina y fibrinógeno) en el suero no se pudo demostrar debido al reducido tamaño de la muestra.
