Abstract
Las estatinas son los fármacos de elección en el tratamiento de hipercolesterolemia aislada o hiperlipidemias combinadas. Son fármacos de elevada eficacia y con un buen perfil de seguridad aunque no quedan exentos de efectos secundarios, miotoxicidad y hepatotoxicidad, a consecuencia de sus posibles interacciones en el uso concomitante con otros fármacos especialmente en el paciente polimedicado. Se ha realizado una extensa revisión bibliográfica con el objetivo de obtener una comparación en el perfil de seguridad de las estatinas. Dicha revisión nos permitió ver como las posibles interacciones no son iguales en todas las estatinas, si no que van acorde a su farmacocinética individual. A nivel de la absorción pitavastatina y rosuvastatina presentaron una mejoría respecto a sus compañeras de grupo al poder administrarse indistintamente con o sin comida permitiendo una mejor adherencia al tratamiento por parte del paciente. La distribución no presentó problema significativo en estos fármacos, siendo su metabolismo el punto crítico de estudio en el perfil de seguridad. Simvastatina, atorvastatina, y lovastatina fueron las que más interacciones potenciales presentaron al metabolizarse por la isoenzima CYP3A4 del citrocomo-p450 al igual que un elevado número de fármacos. Les siguió la fluvastatina con un menor porcentaje de interacciones al metabolizarse por un isoenzima menos habitual, el CYP2C9. Pravastatina, pitavastatina y rosuvastatina por último fueron las que mejor perfil de seguridad presentaron al no metabolizarse por citocromo p450. Es necesario un estudio previo, individualizado y riguroso del paciente con el fin de realizar una prescripción adecuada para evitar posibles efectos adversos.
Statins are the drugs of choice in the treatment of isolated hypercholesterolemia or combined hyperlipidemia. Statins are drugs with a high efficacy and a good safety profile but not without side effects, myotoxicity and hepatotoxicity, as a result of their possible interactions with concomitant use with other drugs especially in a polymedicated patient. It has been done an extensive literature review in order to obtain a comparison in the safety profile of statins. This review allowed us to see how possible interactions are not equal in all statins, they are produced according to their individual pharmacokinetic Pitavastatin and rosuvastatin presented a better absortion as they can be administrated either with or without food, allowing a better adherence to treatment by the patient. Distribution was not a significant problem in these drugs, and metabolism´s study was the critical point in the safety profile. Simvastatin, atorvastatin, and lovastatin had the most potential interactions as they are metabolized by the CYP3A4 isoenzyme of citrocomo-p450 as a large number of drugs. They were followed by fluvastatin, with a lower percentage of interactions as it´s metabolized by a less common isoenzyme CYP2C9. Pravastatin, pitavastatin and rosuvastatin had the better safety profile as they aren´t been metabolized by cytochrome p450. It´s necessary a prior, individual and thorough study of the patient in order to conduct a proper prescription to avoid possible adverse effects.
