Abstract
El síndrome del túnel del carpo (STC) es una de las neuropatías periféricas más comunes que afectan a la población. En la mayoría de los pacientes, la causa exacta y la fisiopatología del STC no es del todo conocida. En diferentes perfiles profesionales se ha observado una mayor incidencia y prevalencia del STC. El riesgo de padecerlo es alto en ocupaciones que involucran una exposición a altas presiones, mucha fuerza, trabajo repetitivo y herramientas vibratorias. Los síntomas clásicos del STC incluyen dolor nocturno asociado con hormigueo y entumecimiento en la distribución del nervio mediano en la mano. Los pacientes con síntomas leves pueden mejorar con un tratamiento conservador, especialmente la inyección local de esteroides o la utilización de férulas de reposo. Sin embargo, en casos moderados o severos, o con debilidad motora tenar, la cirugía parece ser el único tratamiento que ofrece mejorías clínicas. Las técnicas de movilización neurodinámica (MND) son una opción de tratamiento que podría mejorar los síntomas de los pacientes con STC, no obstante, existe poco conocimiento sobre su efectividad terapéutica. El objetivo de este trabajo es mostrar una revisión sobre la efectividad de los ejercicios de deslizamiento del nervio mediano en el STC. En los estudios analizados no se encuentran resultados concluyentes que sean favorables en cuanto a la efectividad de la MND, mostrando una tendencia a posible mejoría. La MND, si se combina con otros tratamientos conservadores, parece tener un efecto superior. Se necesitan más investigaciones que respalden estos efectos positivos.
Carpal tunnel syndrome (CTS) is one of the most common peripheral neuropathies affecting the population. In most patients, the exact cause and pathophysiology of CTS is not well known. In different professional profiles, a higher incidence and prevalence of CTS has been observed. The risk of suffering from it is high in occupations that involve exposure to high pressures, a lot of force, repetitive work and vibratory tools. The classic symptoms of CTS include nocturnal pain associated with tingling and numbness in the distribution of the median nerve in the hand. Patients with mild symptoms may improve with conservative treatment, especially local steroid injection or the use of resting splints. However, in moderate or severe cases, or with tenar motor weakness, surgery seems to be the only treatment that offers clinical improvements. Neurodynamic mobilization techniques (NMT) are a treatment option that could improve the symptoms of patients with CTS, nevertheless, there is little knowledge about their therapeutic effectiveness. The objective of this work is to show a review about the effectiveness of median nerve sliding exercises in the CTS. In the studies analyzed, there are no conclusive results that are favorable in terms of the effectiveness of the NMT, showing a tendency to possible improvement. If NMT is combined with other conservative treatments, seems to have a superior effect. More research is needed to support these positive effects.
