Abstract
Los trastornos del metabolismo de la vitamina D en niños con insuficiencia renal crónica (IRC) constituyen uno de los factores claves en la prevención de la osteodistrofia renal y en el retraso del crecimiento característico de estos pacientes. El fin de la terapia con calcitrol o de su análogo (1-alfa calcidol) en la IRC leve, moderada y avanzada es revertir las manifestaciones tempranas del hiperparatiroidismo secundario y de esta forma prevenir el desarrollo de un hiperparatiroidismo severo o irreversible previo a la diálisis. El objetivo de este trabajo ha sido estudiar en niños con diferente grado de insuficiencia renal el efecto que el tratamiento con dos derivados de la vitamina d produce en los parametros bioquímicos utilizados en el control del metabolismo fosfocalcico, en la incidencia de hipercalcemia y en el control de la funcion renal, asi como en el ritmo del crecimiento. Finalmente, se puede concluir que los dos derivados de la vitamina D utilizados consiguen un buen control del hiperparatiroidismo secundario y la incidencia de hipercalcemia es más elevada en aquellos grupos con mayor grado de insuficiencia renal y menos hiperparatiroidismo. Este tratamiento no acelera el deterioro de la función renal y el empleo de calcitriol incrementa la velocidad de crecimiento en los niños con insuficiencia renal avanzada y niveles de PTH basales inferiores a 100
Disorders of vitamin D metabolism in children with chronic renal failure (CRF) constitute one of the key factors in the prevention of renal osteodystrophy and in the growth retardation characteristic of these patients. The purpose of therapy with calcitrol or its analog (1-alpha calcidol) in mild, moderate and advanced CRF is to reverse the early manifestations of secondary hyperparathyroidism and thus prevent the development of severe or irreversible hyperparathyroidism prior to dialysis. . The objective of this work has been to study in children with different degrees of renal failure the effect that the treatment with two derivatives of vitamin D produces in the biochemical parameters used in the control of phosphocalcic metabolism, in the incidence of hypercalcemia and in the control kidney function, as well as the rate of growth. Finally, it can be concluded that the two vitamin D derivatives used achieve good control of secondary hyperparathyroidism and the incidence of hypercalcemia is higher in those groups with a greater degree of renal failure and less hyperparathyroidism. This treatment does not accelerate the deterioration of renal function and the use of calcitriol increases the growth rate in children with advanced renal failure and basal PTH levels below 100
