Estudio clínico prospectivo a 5 años de puentes posteriores de tres unidades de circona

Abstract
INTRODUCCIÓN. En los últimos años, se han desarrollado nuevos materiales cerámicos cuyas propiedades mecánicas han mejorado notablemente, con el fin de proporcionar una mayor longevidad de las restauraciones y mejorar la resistencia a la fractura. OBJETIVOS El objetivo de este estudio fue comparar las tasas de supervivencia clínica y las complicaciones biológicas y mecánicas de puentes posteriores metal-cerámicos de tres unidades con puentes confeccionados con un sistema de óxido de circonio, IPS e.max ZirCAD®. La hipótesis nula fue que no se encontrarían diferencias en los parámetros estudiados entre los dos tipos de restauraciones. MATERIAL Y MÉTODO Se incluyeron en el estudio 36 pacientes, en los cuales estaba indicada la colocación de una prótesis parcial fija (PPF) de 3 unidades con sustitución de un premolar o un molar, realizándose un total de 40 PPF posteriores de 3 unidades, que fueron asignadas de forma aleatoria, 20 circona y 20 metal-cerámica. La preparación de los pilares se realizó de la siguiente manera: la reducción axial fue de 1 mm de profundidad y con un ángulo de entre 10 y 12 grados, y una reducción oclusal de 1,5 a 2 mm. La línea de terminación fue en chámfer. Las impresiones se realizaron con una silicona de adición, Express Penta Putty® (3M ESPE). Las restauraciones provisionales se realizaron con una resina acrílica Protemp Garant, (3M ESPE) y se cementaron utilizando un cemento de óxido de zinc-eugenol temporal, Integrity TempGrip® (Dentsply). Las restauraciones cerámicas se realizaron utilizando un sistema CAD/CAM, IPS e.max ZirCAD ® (Ivoclar Vivadent) y las de metal-cerámica fueron fabricadas con una aleación de cromo-cobalto (Heraenium Pw, Heraeus Kulzer) siguiendo la técnica convencional. Las PPF de circona se cementaron utilizando un cemento de resina de polimerización dual RelyX Unicem® (3M ESPE) y las de metal-cerámica fueron cementadas utilizando un cemento de vidrio ionómero Ketac Cem® (3M ESPE)...
INTRODUCTION. The aesthetic demands of both patients and dentists have increased, even for posterior teeth, since all-ceramic restorations are becoming more popular and natural looking. The most recently used core material for all-ceramic FDPs is zirconium oxide, especially with yttrium oxide added. Zirconium dioxide exhibits excellent mechanical properties with high fracture strength and high fracture toughness. The zirconium dioxide frameworks are manufactured primarily with computer-aided design and computer-aided manufacturing (CAD/CAM) technology, and studies have demonstrated an adequate marginal integrity for the zirconium dioxide FDP frameworks with or without its corresponding veneering porcelain. Metal-ceramic restorations, also known as porcelain-fused-to-metal (PFM) restorations, are considered the gold standard in fixed prosthodontics. All new materials used as an alternative have to be as good as metal-ceramic, particularly with regard to veneer chipping, core fracture and marginal fit. New zirconia ceramics provide the possibility of producing FDPs in posterior teeth with no restrictions on concerning the tooth to be restored. Results of in vitro and clinical studies suggest that this material is a promising alternative for the production of FDPs in posterior regions. However, with all their favourable mechanical, biological, and aesthetic properties, zirconium dioxide-based FDPs have not been free of clinical complications, being the chipping of the veneering ceramic one of the main problems encountered. Several studies have shown metal-ceramic restorations to have 5-, 10-, and 20-year survival rates of approximately 95% to 98%. 90% and 41% to 73%, respectively. In contrast, studies on ceramic FDPs have shown rather lower survival rates. In light of the fact that the results of clinical trials suggest that zirconia restorations may replace metal-ceramic ones in the near future, more clinical studies, comparing both types of restorations, are needed with a longer observation period and with different zirconia systems...
