Biopelículas orales en superficies de titanio y zirconio: respuesta a agentes antisépticos.

Abstract
Objetivos: Se ha demostrado recientemente que las biopelículas in vitro formadas en diferentes superficies de implantes son significativamente diferentes en cuanto a su espesor, en la organización estructural de sus bacterias y a la disposición de la matriz de polisacárido extracelular. El objetivo de este estudio fue evaluar si estas diferencias conllevan una respuesta diferente de las bacterias a la exposición de diferentes agentes antisépticos. Material y Métodos: Se utilizaron seis cepas bacterianas de referencia para desarrollar una biopelícula in vitro sobre discos estériles de titanio y de zirconio, recubiertos de saliva, dentro de los pocillos de placas de cultivo celular. Las especies seleccionadas representan colonizadores iniciales (Streptococcus oralis y Actinomyces naeslundii), tempranos (Veillonella parvula), secundarios (Fusobacterium nucleatum) y colonizadores tardíos (Porphyromonas gingivalis y Aggregatibacter actinomycetemcomitans). Los agentes antimicrobianos: clorhexidina combinado con cloruro de cetilpiridinio (CHX/CPC), aceites esenciales (EO) y cloruro de cetilpiridinio (CPC) se aplicaron a biopelículas de 72 h de desarrollo, mediante la inmersión de los discos durante 1 min en las soluciones antisépticas y en solución tampón de fosfato (PBS) como control, bajo agitación constante a 90 rpm o en ausencia de ella. Se estudió antes y después de la exposición el número de bacterias viables (unidades formadoras de colonias UFC/mL) de las especies bacterianas F. nucleatum, P. gingivalis y A. actinomycetemcomitans mediante PCR cuantitativa a tiempo real (qPCR) combinada con Propidio de Monoazida (PMA), y se realizó un modelo lineal generalizado para A. actinomycetemcomitans, P. gingivalis y F. nucleatum, con el fin de determinar el efecto de cada variable independiente, aislada o interrelacionadas, en el recuento de bacterias viables (UFC/mL). Resultados: No se observaron diferencias significativas en el número de bacterias viables (UFC viables/mL) para las tres cepas seleccionadas cuando se comparó la respuesta bacteriana en superficies de titanio y zirconio después de cada uno de los tratamientos antimicrobianos aplicados (CHX/CPC, EO y CPC), ya sea con o sin agitación. Asimismo, ambas biopelículas respondieron de forma similar al tratamiento mecánico, a pesar de la exposición a las soluciones antisépticas. Conclusiones: Tres de la principales especies bacterianas asociadas a las enfermedades peri-implantarias (A. actinomycetemcomitans, P. gingivalis y F. nucleatum) respondieron de manera similar frente a diferentes antisépticos cuando fueron expuestas a los mismos en forma de biopelículas formadas sobre dos superficies de implantes diferentes. Esto ocurrió independientemente de las diferencias de esas biopelículas en cuanto a su espesor, en cuanto a la disposición de la matriz de polisacárido extracelular, o en cuanto en la organización de las células bacterianas en la biopelícula, asociadas a las diferentes características de las superficies.
