Abstract
Los trastornos de la conducta alimentaria (TCA) son uno de los problemas de Salud Pública más graves de nuestros días, tanto por su prevalencia y necesidad de tratamiento especializado, como por su tendencia a la cronicidad y la gravedad (Oliva et al., 2011). Una de las características principales de los TCA es la migración diagnóstica, ya que, mientras que un pequeño subgrupo de pacientes se mantiene en el síndrome inicial con el que comenzó, la gran mayoría muda o vira entre los diferentes trastornos. Los TCA se han relacionado con sentimientos de disforia, depresión y sintomatología ansiosa, junto con una autovaloración/autoestima baja, lo que a su vez repercute en las relaciones interpersonales y puede dificultar el afrontamiento de acontecimientos vitales estresantes (AVE). Además, existe una importante línea de investigación que ha analizado las diferentes agrupaciones posibles en relación con los rasgos de personalidad en sujetos con TCA. El objetivo general de la presente tesis es estudiar las variables clínicas asociadas con los trastornos de la conducta alimentaria en una muestra de participantes del programa de trastornos de la conducta alimentación del dispositivo de Interconsulta y Enlace del Hospital Universitario de Guadalajara. Todos los participantes han sido evaluados y diagnosticados de algún TCA por un facultativo especialista (psicólogo clínico o psiquiatra). La muestra se compone de 118 pacientes, todas ellas mujeres, con edades comprendidas entre los 18 y los 59 años, y una edad media de 31,90 (DT = 10,23). La evaluación se realiza a través de un protocolo de evaluación para pacientes con TCA, recogiéndose también información sobre acontecimientos vitales estresantes a través de entrevista y diversos instrumentos (MCMI-III, EDI-2, PBI, STAI, BDI, ASI-3, RAS, TAS-20, AQ, EI, NHP, DUKE, EAR). Eating disorders (ED) are one of the most serious public health problems of our time, because of their prevalence, because they need specialized treatment, and because of their tendency towards chronicity and severity (Oliva et al., 2011). One of the main features of ED is diagnostic migration; while a small subset of patients remains in the initial syndrome, the vast majority change or turn between different disorders. ED have been associated with feelings of dysphoria, depression, and anxious symptomatology, along with low self-esteem, which in turn affect interpersonal relationships and may make coping with stressful life events difficult. In addition, there is an important line of research that has analyzed the different possible clusters in relation to the personality traits in subjects with ED. The general objective of this thesis is to study the clinical variables associated with ED in a sample of participants in the Eating Disorders Program of the Consultation-Liaison Unit of the Guadalajara University Hospital. All participants have been evaluated and diagnosed with ED by a specialist (clinical psychologist or psychiatrist). The sample consists of 118 patients, all of them women, aged between 18 and 59 years, with a mean age of 31.90 (SD = 10.23). The evaluation is carried out using a protocol for patients with ED, which includes information collection on stressful life events through interview and several instruments (MCMI-III, EDI-2, PBI, STAI, BDI, ASI-3, RAS, TAS-20, AQ, EI, NHP, DUKE, EAR). Statistical analyzes are performed using SPSS 19.0.
