Abstract
Objetivo: evaluar cuantitativamente parámetros topográficos y la transparencia corneal mediante la medición de la densitometría corneal con la topografía Scheimpflug (Pentacam) en pacientes con queratocono (KC) sin tratamiento previo y sujetos control sanos (N), comparándolos a su vez entre sí. Material y método: se realizó un estudio trasversal, controlado no aleatorizado. La muestra consta de 50 pacientes con queratocono y 50 pacientes control, a los cuales se les realizó una medida mediante el sistema topográfico Pentacam en la Unidad de Superficie Ocular (USIO) del Hospital Clínico San Carlos. Mediante esta medida analizamos la densitometría corneal, la paquimetría central (CCT), la paquimetría más fina, la queratometría simulada (SimK), la queratometría total (TotalK) y la profundidad de la cámara anterior (ACD). Junto con ello, en la misma visita, se midió la refracción (Rx) y la mejor agudeza visual corregida para visión lejana (CDVA). Resultados: encontramos diferencias estadísticamente significativas (p=0.000) en la densitometría corneal principalmente en los anillos centrales de 6 mm (N=16,47±0,68; KC=18,76±3,41 en el anillo central 2 mm del total de las capas). Al igual que ocurría con SimK (N=43,21±1,36; KC=47,34±3,39), TotalK (N=42,94±1,38; KC= 46,40±2,63), CCT (N=548,68±29,65; KC= 482,46±38,38), paquimetría más fina (N=544±29,93; KC= 462,04±38,34), CDVA (N=-0,02±0,10; KC=0,27±0,24) y el equivalente esférico (N=-0,97±2,17; KC=-3,64±2,92). También se encontraron diferencias significativas al analizar la influencia de la edad, respectivamente en cada grupo, en la densitometría corneal principalmente en los anillos periféricos (N(18-29)=16,30±0,98, N(50-60) = 23,80±4,58 (p=0.000); KC(18-29)=17,27±1,87, KC(50-60)=20,71±3,45 (p=0.012), en el diámetro total del total de las capas) Conclusiones: el queratocono aumenta los valores de la queratometría simulada (SimK) y la queratometría total (TotalK), disminuye la paquimetría central (CCT) y la paquimetría más fina, aumenta la profundidad de la cámara anterior (ACD), disminuye la mejor agudeza visual corregida para visión lejana (CDVA) y aumenta la densitometría corneal, es decir, disminuye la transparencia corneal, principalmente en los anillos centrales de las capas de la córnea. Además, en córneas tanto sanas como queratocónicas, la edad aumenta la densitometría corneal particularmente en los anillos periféricos.
Purpose: to evaluate quantitatively topographic parameters and corneal transparency by measuring corneal densitometry with Scheimpflug topography (Pentacam) in patients with keratoconus (KC) without previous treatment and healthy control subjects (N), comparing them to each other. Material and method: a cross-sectional, nonrandomized controlled study was designed. The sample consisted of 50 patients with keratoconus and 50 control patients, who underwent a measurement using the Pentacam topographic system in the Ocular Surface Unit of the San Carlos Clinical Hospital. Through this measure we analyze corneal densitometry, central pachymetry (CCT), thinner pachymetry, simulated keratometry (SimK), total keratometry (TotalK) and depth of the anterior chamber (ACD). Along with this, in the same visit, the refraction and the best corrected visual acuity for distant vision (CDVA) were measured. Results: we found statistically significant differences (p=0,000) in the corneal densitometry mainly in the central rings of 6 mm (N=16,47±0,68, KC=18,76±3,41 in the central ring 2 mm of the total the layers). As was the case with SimK (N=43,21±1,36, KC=47,34±3,39), TotalK (N=42,94±1,38, KC=46,40±2,63), CCT (N=548,68±29,65; KC=482,46±38,38), finer pachymetry (N=544±29,93, KC=462,04±38,34), CDVA (N)=-0,02±0,10, KC=0,27±0,24) and the spherical equivalent (N=-0,97±2,17, KC=-3,64±2,92). Significant differences were also found when analyzing the influence of age, respectively in each group, on corneal densitometry mainly in the peripheral rings (N(18-29)=16,30±0,98, N(50-60)=23,80±4,58 (p=0.000), KC(18-29)=17,27±1,87, KC(50-60)=20,71±3,45 (p=0,012), in the total diameter of the total layers) Conclusions: keratoconus increases the values of simulated keratometry (SimK) and total keratometry (TotalK), decreases the central pachymetry (CCT) and the pachymetry finer, increases the depth of the anterior chamber (ACD), decreases the best corrected visual acuity for far vision (CDVA) and corneal densitometry increases, that is, corneal transparency decreases, mainly in the central rings of the layers of the cornea. In addition, in both healthy and keratoconic corneas, age increases corneal densitometry particularly in the peripheral rings.
