Abstract
La toma de impresiones en prótesis sobre implantes supone todo un reto para el profesional e investigador. Con la llegada de la era digital, la tecnología CAD/CAM ha provocado una revolución en esta área. La precisión (entendida como ajuste) ha sido una condición buscada a lo largo de la historia en todo tipo de rehabilitaciones en el ámbito de la prostodoncia. A día de hoy sigue siendo objeto de estudio y de mejora. Por otra parte, la ausencia de ligamento periodontal alrededor de los implantes limita su movimiento. esta dificultad inherente al sistema obliga a fabricar unas excelentes rehabilitaciones, para evitar posibles complicaciones por desajuste de la prótesis. También es necesario valorar la importancia del tiempo y la paradoja que conlleva. A día de hoy, existe una corriente muy potente en la que se reivindica la importancia de una Odontología "lenta", con tiempos necesarios y enfocado a un trato excelente al paciente. Estono significa que haya que quitarle valor al tiempo consumido, ni que una impresión de calidad tenga que ser más lenta. Precisamente, de eso se trata. Dedicar el tiempo a lo que realmente lo necesita, y optimizarlo para todo lo demás. Estas demandas son las que esta Tesis Doctoral pretende satisfacer, que no sólo son del paciente cuando necesita terminar cuanto antes con esa incómoda impresión, sino que también el odontólogo requiere terminar lo más pronto posible, asegurándose por supuesto un resultado inmejorable. Desde esta Tesis Doctoral se reivindica la importancia de una buena toma de impresiones para el éxito de la rehabilitación, y no sólo de la cirugía o el material utilizado para ello. Se debe destacar la falta de consenso a la hora de llevar a cabo las mediciones, interpretar los resultados de precisión (que según la ley ISO 5725-1:1994 se engloba dentro del mismo término exactitud) por parte de la comunidad científica, junto a su análisis estadístico. Después de ello, se puede decir que los resultados obtenidos son variados. Los cuatro sistemas de medición estudiados, presentaron diferencias estadisticamente significativas entre ellos, con respecto a los tiempos de trabajo utilizados. Con respecto al grupo control, el sistema PiC ofrece la mejor precisión con respecto a la posición de los implantes, con , con unas cifras muy inferiores de desviación estándar y discrepancia promedio al resto de sistemas estudiados. Se debe destacar que este sistema nopermite exportar los archivos si la obtención de datos no ha sido al 100%. Sin embargo, este factor no se considera de especial relevancia, ya que el tiempo medio de trabajo de PiC es considerablemente más bajo que el resto de sistemas. Continuando con el análisis , el escáner CS3600 presenta la menor desviación estándar y discrepancia promedio al 100% del tiempo analizado. Sin embargo estos valores empiezan a aumentar progresivamente a medida que se disminuyen los tiempos de trabajo. Este sistema presenta las mayores discrepancias cuando el tiempo se disminuye al máximo. Es decir, a medida que se disminuyen los tiempos, pierde precisión. Los sistemas TRIOS 3 y MEDIT i500 presentan resultados más similares y estables, siendo mejores en el sistema MEDIT i500 cuando los tiempos disminuyen. Se considera de especial interés , destacar algunas diferencias encontradas en la bibliografía con respecto a ciertos aspectos en la Metodología utilizada en otros estudios. Estos aspectos son la Metodología utilizada para la obtención y análisis de datos y las magnitudes utilizadas para el análisis estadístico. Desde esta Tesis Doctoral, se pretende crear una nueva línea de Investigación basada en el estudio de esos aspectos.
The main requirement of the implants supported prostheses is to take an accurate impression to obtain the exact position of the implants during the processing of the definitive model. Although conventional impression has been the gold standard of practice for many decades, with the advent of the digital era, CAD/CAM technology has brought about a revolution in this area. Precision (understood as fit) has been a condition sought throughout the history of all types of rehabilitation in the field of prosthodontics. Nowadays, it is still the object of study and improvement. On the other hand, the absence of periodontal ligament around the implants limits their movement. This inherent difficulty in the system forces us to produce excellent rehabilitation, to avoid possible complications arises from prosthetic mistfit. It is also necessary to assess the importance of time and the paradox what it entails. Today, there is a very powerful trend in which the importance of a "slow" dentistry is claimed, with necessary times and focused on an excellent treatment to the patient. This does not mean that you have to take away value from the time consumed, or that the quality of impression has to be slower. Precisely, it is about dedicating time towhat really needs it, and to optimize it for everything else. These demands are the ones that this Doctoral Thesis aims to satisfy, which are not only of the patient when you need to finish as soon as possible with that uncomfortable impression, but also the dentist needs to finish as soon as possible, ensuring an unbeatable result. From this Doctoral Thesis the importance of a good impression taking for the success of the rehabilitation, and not only of the surgery or the material used for it, is claimed. It should be highlighted the lack of consensus when carriying out the measurements, interpreting the precision results (according to the law ISO 5725 is included within the term accuracy) on the part of the scientific community, together with its statistic analysis. In the rpesent study it can be said that the results obtained are varied. the four optical measurement systems studied showed statistically significant differences among them, with respect to the work times used. Comparing to the control group, the PiC system offers the best accuracy with respect to the position of the implants, with much lower figures of standard deviation and average discrepancy to the rest of the systems studied. It should be mentioned that this system does not allow exporting the files if the data collection has not been 00%. however, this factor is not considered special relevance, since PiC's average working time is considerably lower than the rest of the systems. The CS3600 scanner presents the smallest standard deviation and average discrepancy at 100% of the time analyzed. However, these values begin to increase progressively as work times decrease. This system has the greatest discrepancies when time is reduced to the maximum, that is to say, as the time decrease, it loses precision. The TRIOS 3 and MEDIT i500 systems present more similar and stable results, being better in the MEDIT i500 system when times are reduced, It is considered of special interest to hightlight some differences found in the bibliography with respect to certain aspects in the Methodology used in other studies. These aspects are the Methodology used to obtain and analyze data and the magnitudes used for statistical analysis. From this Doctoral Thesis,we want to create a new Research line based on the study of those aspects.
